Nevada prescribes 4 statutory lien waiver forms (Nevada Revised Statutes 108.2457). This page lists each one, when a subcontractor signs it, what the statute requires on the page and the blanks it has, with a form that fills the statutory text with your figures and downloads the PDF.

The forms the statute prescribes

Conditional Waiver and Release upon Progress Payment (NRS 108.2457(5)(a)). Send it with the pay app. It takes effect when the check for the payment amount is paid.

Unconditional Waiver and Release upon Progress Payment (NRS 108.2457(5)(b)). Sign it after the progress payment has cleared. It binds you to the extent of the amount received.

Conditional Waiver and Release upon Final Payment (NRS 108.2457(5)(c)). Send it with the final pay app. It keeps the disputed claims you enter.

Unconditional Waiver and Release upon Final Payment (NRS 108.2457(5)(d)). Sign it after the final payment has cleared.

What Nevada Revised Statutes 108.2457 requires

A written waiver is enforceable only in the statutory form, signed by the lien claimant or an authorized agent. A conditional waiver also needs receipt of the amount it names (subsection 1).

Each unconditional form carries the statutory notice in type at least as large as the largest type on the document.

If the check given for any waiver fails to clear, the waiver is void (subsection 5(e)).

A progress waiver covers the invoice or pay application it names, only to the extent of the payment amount, and keeps retention, pending changes and disputed items.
