The forms the statute prescribes
Partial Conditional Waiver (MCL 570.1115(9)(b)). Send it with the pay app. It takes effect on payment of the amount shown.
Partial Unconditional Waiver (MCL 570.1115(9)(a)). Sign it after the progress payment has cleared, for the amount received.
Full Conditional Waiver (MCL 570.1115(9)(d)). Send it with the final pay app. It takes effect on payment of the amount you enter.
Full Unconditional Waiver (MCL 570.1115(9)(c)). Sign it after the contract has been paid in full.
What Michigan Compiled Laws 570.1115 requires
Four forms in subsection 9: partial unconditional, partial conditional, full unconditional and full conditional.
A claimant paid in full must give a full unconditional waiver. One paid in part gives a partial unconditional waiver for the amount received, on request (subsections 2 and 3).
A conditional waiver takes effect on payment of the amount it shows (subsection 4).
Each form carries the residential structure paragraph: an owner who did not receive the waiver from you directly must verify it with you before relying on it.
The partial forms ask whether this waiver, together with all earlier ones, covers all amounts due through the date shown.

Partial Conditional Waiver: Customer (who you contracted with), Work or materials you furnish, Property description, Payment amount, Through date, Covers all amounts due through the date (yes or no), Claimant (your company), Date signed, Address, Telephone.
Partial Unconditional Waiver: Customer (who you contracted with), Work or materials you furnish, Property description, Payment amount, Through date, Covers all amounts due through the date (yes or no), Claimant (your company), Date signed, Address, Telephone.
