Florida prescribes 4 statutory lien waiver forms (Florida Statutes section 713.20). This page lists each one, when a subcontractor signs it, what the statute requires on the page and the blanks it has, with a form that fills the statutory text with your figures and downloads the PDF.
The forms the statute prescribes
Waiver and Release of Lien upon Progress Payment, conditioned on the check (section 713.20(4) with 713.20(7)). Send it with the pay app before payment. It takes effect when the check is paid.

Waiver and Release of Lien upon Final Payment, conditioned on the check (section 713.20(5) with 713.20(7)). Send it with the final pay app before payment.
Waiver and Release of Lien upon Final Payment (section 713.20(5)). Sign it after the final payment has cleared.
What Florida Statutes section 713.20 requires
The statute gives a progress form (subsection 4) and a final form (subsection 5). Nobody may require a lienor to sign a different form (subsection 6).
A lienor who signs a waiver in exchange for a check may condition it on payment of the check (subsection 7). The conditional versions here add one sentence that says so; the statute prescribes no wording for it, so read that sentence before you sign.
The progress form releases labor, services and materials furnished through the date you enter and keeps retention and later work.
A waiver that is not substantially similar to the statutory forms is enforced on its own terms (subsection 8), so stay with the form.
