Arizona prescribes 4 statutory lien waiver forms (Arizona Revised Statutes section 33-1008). This page lists each one, when a subcontractor signs it, what the statute requires on the page and the blanks it has, with a form that fills the statutory text with your figures and downloads the PDF.

The forms the statute prescribes

Conditional waiver and release on progress payment (A.R.S. 33-1008(D)(1)). Send it with the pay app. It takes effect when the named check is endorsed and paid.

Unconditional waiver and release on progress payment (A.R.S. 33-1008(D)(2)). Sign it after the progress payment has cleared.

Conditional waiver and release on final payment (A.R.S. 33-1008(D)(3)). Send it with the final pay app. It keeps disputed claims in the amount you enter.

Unconditional waiver and release on final payment (A.R.S. 33-1008(D)(4)). Sign it after the final payment has cleared.

What Arizona Revised Statutes section 33-1008 requires

A waiver is effective only if it follows substantially one of the four forms in subsection D and is signed by the claimant or an authorized agent. A conditional release also needs evidence of payment.

Each unconditional waiver carries the statutory notice in type at least as large as the largest type on the document.

A progress waiver covers work through the date you enter and never covers retention, pending changes or items furnished after that date.

Each form carries a warranty that the money received goes to your own laborers, subcontractors and suppliers.
